Position Description The Patient Flow Coordinator will be responsible for the delivery of patient care by providing a high level of support and assistance to patient and families and the team in the Nephrology Outpatient Clinic. They will support the operation of the clinic by ensuring patients and families flow smoothly and efficiently through their visit in the clinic in a proactive and organized manner. Here’s What You’ll Get To Do Greet patients and families and orient them to the clinic Support the flow of patients and families starting from registration through to the end of their visit Supporting rooming tasks for patients with support of RN team Support the Ambulatory Blood Pressure Monitoring program alongside the RN team Work closely with the clinical team to ensure smooth and seamless daily operations of the clinic Support clinical staff in organizing and storing materials that are utilized in the clinic Facilitate communication flow via telephone, paging, and computer access Reschedule appointments as necessary and inform necessary people of these changes Collate clinical data and relay this information in a precise and timely manner to colleagues within and outside of the organization Support the clerical staff with the registration and flow of patients within the clinic Maintain an efficient and tidy work environment Initiate and assist in the coordination of family/patient and health care professional education sessions and events Communicate with inter-professional team members within and outside of the organization to coordinate the booking visits Provide registration coverage as required Here’s What You’ll Need Bachelor’s Degree in a related field (Medical Administration, Business Administration, Health Administration) preferred Minimum of 2 years clerical or administrative experience in a health care setting. Experience in a hospital environment is preferred Scheduling experience is essential and medical terminology is an asset Accurate keyboarding skills and strong working knowledge of Word, Excel, PowerPoint, and Outlook Experience with Epic is an asset Excellent problem solving skills that demonstrate an ability to anticipate potential issues and make suggestions Strong interpersonal, communication and organizational skills with both fellow colleagues and patients Ability to remain focused and prioritize tasks in a busy and high stress environment Dependability and good attendance Employment Type; Full time, permanent Our Commitment to Diversity The children and families we care for are from a variety of communities, and so are our employees.
